Parent and Student Notification Flyer

Use this page to download the flyer that is to be sent to parents by September 26, 2008 (that is, at least two weeks prior to the survey administration). Schools must send the flyer using one of the approved methods. Each participating school indicated on the school registration form which method will be used to send the flyer to parents.

For the Healthy Youth Survey 2008 there is only one version of the flyer for notifying parents and students.
