Thank you for your interest in the Healthy Youth Survey 2008! RMC
Research sent all superintendents in Washington State a packet of
information on the Healthy Youth Survey in March 2008.
This same packet was also sent to all principals in schools with
students in Grades 6, 8, 10, or 12.
To register a school for the survey, complete the registration
form included in that packet and mail or fax it to RMC Research
no later than June 26, 2008.
The registration form for each school must include the principal's
signature and all requested information. We cannot register your
school if your form is incomplete.
You may request a new registration form through one of the
sponsoring agencies,
your
ESD Coordinator, or
RMC Research.
Tips on completing the registration form:

You must complete a separate registration form for
each school you wish to register.

Be sure to fill out the entire form. All information
is required, including the number of students taking the survey.

An e-mail address for the coordinator is essential so we can
contact you readily with questions we may have.

Your shipping address must be a physical street address. UPS
will not deliver to a PO Box.

Be sure to print your district and school names clearly.
