How to View a Copy of the Survey

Review copies of the surveys were sent with the recruitment materials to all district superintendents and to principals of all schools with an eligible grade (6, 8, 10, or 12). To arrange to view a copy, please contact your superintendent or the principal of a school in your area with an eligible grade.

The Health Youth Survey 2006 may be downloaded from this page. The Healthy Youth Survey 2008 will be similar to the 2006 version.
